Brown the sausage in a large Dutch oven over medium heat for 5-7 minutes, then drain excess grease.
Add diced onion, Italian seasoning, salt, and pepper; cook for 1-2 minutes until the onion is translucent.
Stir in minced garlic and cook for an additional minute.
Pour in diced tomatoes, chicken broth, and ditalini pasta; bring to a boil and cook for about 8 minutes until pasta is al dente.
Reduce heat to low, stir in cream cheese and Parmesan, and cook for about 5 minutes until melted.
Ladle soup into bowls and garnish with fresh basil before serving.
